Exercise Support After Referral

A Defined Role in the Healthcare Team

EverStrong provides medical-supportive fitness coaching for patients whose training is guided by a healthcare provider.


You establish the medical guidance. We adhere to it throughout the program.


Praxis assesses the athlete, coaches the work and provides objective information back to the healthcare team through an established referral reporting loop.

When to Refer

EverStrong may be appropriate when exercise has been prescribed as part of a patient's care, a patient is transitioning from physical therapy, participating in medical weight management, or has a condition that establishes specific boundaries for training.


EverStrong does not replace medical care, physical therapy, or treatment.


Our role begins with the fitness work.

What Happens After a Referral

We begin with an assessment to establish the athlete's current strength, movement, balance and capacity.


Your orders, restrictions and recommendations establish the boundaries of training.


The athlete's goals give us direction within those boundaries.


From there, we coach.

Coaching Within Your Guidance

EverStrong athletes train three times each week.


Praxis makes the day-to-day coaching decisions within the medical guidance you provide.


Exercises can change. Loads can change. Range of motion can change. Equipment can change. The pace of progression can change.


Your medical guidance does not.


When guidance needs to change, that decision remains with the healthcare provider.

How we Coach

Training That Progresses

Medical guidance establishes what we need to work within. It does not mean the athlete stops progressing.


We select and progress exercises based on the athlete's current ability and goals while adhering to the boundaries you have established.


As ability changes, the training can change with it.

Explore the MEthod

Reassessment and Reporting

The initial assessment establishes a baseline.


We reassess throughout the athlete's time in EverStrong to understand what has changed and how the training should respond.


With the athlete's authorization, reassessment results and relevant fitness session notes can be provided to the referring healthcare provider as part of the established referral reporting loop.


This gives the healthcare team ongoing objective information from the fitness setting while the athlete continues training.

Changes in Medical Guidance

Progress in EverStrong does not override active medical orders or provider-established restrictions.

The athlete remains within those boundaries until updated guidance is received from the healthcare provider.


If restrictions are changed, added or removed, we adjust the training accordingly.


If medical guidance no longer requires the EverStrong pathway, the athlete can then determine what comes next within Praxis.

Our Role

Praxis provides fitness assessment, individualized programming, coaching, progression and reassessment.


We do not diagnose medical conditions, provide medical treatment, prescribe medication or replace licensed healthcare services.


Our responsibility is to coach the athlete within the medical guidance provided and communicate relevant fitness information back to the healthcare team when authorized.
