Praxis Performance Studio LLC ("we," "us," or "our") respects your privacy and is committed to protecting your information. We operate on a foundation of user data sovereignty, transparency, and a strict commitment never to sell your personal information. This Privacy Policy explains how we collect, utilize, and safeguard data submitted through our website, consumer portals, and companion performance platforms. 


2. Operational Scope & Voluntary Data Submission

Unlike standard informational websites, our platform is intentionally designed to receive and manage administrative wellness documentation, performance diagnostics, and individualized metrics to assist you in tracking your fitness narrative. By interacting with our services, portals, or website forms, you acknowledge that you are voluntarily providing personal, biometric, and fitness-related information to optimize your programming. 


3. Non-Covered Entity (NCE) Status & FTC/CCPA Framework

Praxis Performance is not a HIPAA-covered entity or a HIPAA business associate. Information submitted to or stored on this platform is not governed by HIPAA administrative rules. Instead, all consumer wellness data, performance metrics, and personal records collected by us are strictly protected under the Federal Trade Commission (FTC) Health Breach Notification Rule and the California Consumer Privacy Act (CCPA) as amended by the California Privacy Rights Act (CPRA). 


4. Information We Intentionally Collect

To deliver our performance services, we collect the following categories of information:

  • Identifiers: Full name, email address, physical address, and telephone number. 
  • Performance & Biometric Data: Physical assessments, performance diagnostics, coaching logs, and training metrics. 
  • Administrative Wellness Documents: Letters of Medical Necessity (LMNs), associated National Provider Identifier (NPI) numbers, or referral documentation uploaded directly by the user. 
  • Usage Information: Basic digital data, including browser type, pages visited, and IP addresses. 


5. Automated Systems & Technology Tools

We utilize technology-assisted, administrative, and automated systems for operations such as data storage, session documentation, scheduling management, communication processing, and logging metadata. These workflows are optimized exclusively to maintain operational efficiency and document consistency for your performance portfolio. 


6. How We Use Your Information

We utilize collected personal and performance data solely to execute our core business operations, including:

  • Providing, personalizing, and adjusting your coaching, instructional programming, and fitness services. 
  • Managing user accounts, scheduling sessions, processing billing, and verifying qualified rate adjustments. 
  • Responding to administrative inquiries and distributing required legal or operational notices. 
  • Compiling completely anonymized, aggregated longevity and cohort trends, subject to your strict right to opt-out. 


7. Non-Disclosure & Information Sharing

We do not sell your personal or health-related information to any third party. We only share information with third parties in the following strictly defined circumstances: 

  • Service Providers: Infrastructure partners, hosting entities, and payment processors bound by confidentiality rules and HIPAA-aligned security agreements. 
  • Authorized Communications: Designated healthcare providers, provided we have received your explicit written authorization. 
  • Legal Mandates: To satisfy statutory requirements, regulations, or enforceable governmental requests. 


8. Data Security & Limitations

We implement reasonable administrative, technical, and physical safeguards—including technical security arrangements with our infrastructure providers—to protect your records. However, no method of transmission or storage over the internet is completely flawless, and absolute confidentiality cannot be mathematically or legally guaranteed. 


9. Data Retention Parameters

We retain your personal identifiers and performance data only as long as necessary to fulfill your active programming terms, satisfy the operational timelines of your Client Services Agreement, or comply with statutory retention rules mandated by California law. Once information is no longer operationally or legally required, it is securely destroyed or permanently de-identified. 


10. Your Rights & California Sovereignty Choices

Under the CCPA/CPRA, California residents retain explicit rights over their personal information. You may: 

  • Request access to, correction of, or complete deletion of your personal records. 
  • Limit the use of your sensitive personal data or explicitly opt-out of data sharing mechanisms. 
  • Opt-out of promotional media usage or aggregated longevity reports at any time via written notice. 


11. Policy Modifications

We reserve the right to update this Privacy Policy periodically to reflect shifts in regulatory compliance or internal operational realities. Any material changes will be published publicly on this page with a revised "Effective Date" at the top. Continued use of the platform following an update constitutes an acceptance of the revised privacy framework.
